• The ideal candidate has experience supporting patients with personal care needs and daily living activities, while ensuring safety and comfort at every visit.
• **Responsibilities:** --------------------- * Provide personal care assistance such as bathing, grooming, dressing, and toileting.
• * Assist patients with mobility, transfers, and safe ambulation.
• * Support with meal preparation, light housekeeping, and errands as needed.
• * Monitor and report changes in patient condition to supervising nurse.
• * Maintain accurate visit documentation and follow care plan instructions.
• * Deliver compassionate, professional care while respecting patient dignity and privacy.
• **Qualifications:** ------------------- * Active Home Health Aide, CNA, or equivalent certification (depending on state requirements).
• * At least 1 year of experience in home health, caregiving, or a related field preferred.
• * Reliable transportation, valid driver's license, and current auto insurance.
• * Strong communication skills and ability to work independently.
• * Must pass background screening and provide updated immunizations/CPR.
